Medicaid Service Coordination

Our Service Coordinators have earned a reputation as dedicated advocates who work with integrity, creativity and responsiveness and approach their mission with an attitude of warmth, sensitivity, empathy and respect.

HASC Center’s experienced and dedicated Medicaid Service Coordinators (MSC) link individuals and their families with the services they need.

Our multi lingual Service Coordinators meet with individuals and their families in a convenient location. Together, they evaluate the individual's needs for a wide range of services including education, housing, recreation, respite, residential habilitation, clinical services, physical and rehabilitative therapies, transportation services and supported or competitive employment opportunities.

At HASC Center, the Service Coordinator assists individuals and their families to develop a person centered plan based on individual preferences, desires and dreams. The goal is to help each individual attain an integrated role within the community and a meaningful quality of life. This plan is reviewed on a regular basis and adjusted due to ever changing needs. This relationship is built on trust and understanding and ensures that all needs are considered when any minor or significant life change occurs.

Our qualified service coordinators understand the different entitlements and work tirelessly to assist families in the necessary paperwork in order to procure all appropriate services available. They also ensure the maintenance of Medicaid and other government sponsored programs.
